Drug Therapy Problems (DTPs)
What drug therapy problems are, how they are created during an MTR session, and how to track them on the patient's DTPs tab.
A Drug Therapy Problem (DTP) is a clinical issue in a patient's medication regimen that is identified, documented, and tracked to resolution. DTPs are created inside an MTR session during step 3 (Therapy Assessment) — each DTP you log in the session appears on the patient's DTPs tab and in the workspace-wide DTP list.
DTP categories and types
Every DTP is classified by category and a more specific type within it:
| Category | Examples of types within it |
|---|---|
| Unnecessary medication | Duplicate therapy, no indication, treating an adverse effect |
| Needs additional medication | Preventive therapy needed, untreated condition |
| Wrong medication | More effective alternative available, dosage form inappropriate |
| Suboptimal dosage | Dose too low, dose too high, frequency issue |
| Adverse drug reaction | Side effect, allergic reaction, drug interaction causing harm |
| Non-adherence | Patient not taking as directed, cost barrier, patient preference |
Severity and evidence level
Each DTP also carries:
- Severity — Minor, Moderate, or Major — reflecting how significant the clinical impact is
- Evidence level — how confident you are in this finding (Definitive, Probable, Possible)
Logging a DTP
DTPs are created during step 3 of an MTR session. You can't create a standalone DTP outside of an MTR session.
Within an open MTR session, select Add DTP, fill in the category, type, severity, evidence level, and describe the problem in the description field. Optionally, add a proposed resolution.
Once you save it, the DTP appears:
- On the session's own step 3
- On the patient's DTPs tab
- In the workspace-wide DTP list accessible from the sidebar
From DTP to intervention
When a DTP requires an active clinical response (a dose change, a physician referral, a new medication) rather than just documentation, you create a Clinical Intervention from it. In step 5 of the MTR session, select Create intervention next to the DTP — the system creates a linked Clinical Intervention record that has its own independent status workflow and audit trail.
See Clinical Interventions for how to manage interventions.
The patient's DTPs tab
On any patient's record, the DTPs tab shows all of their DTPs across every MTR session — not just the most recent one. Filter by status (Open / Resolved), category, or severity.
Select any DTP to see the full detail: the session it came from, the linked intervention (if any), and the resolution notes.
Workspace-wide DTP list
Go to Clinical Suite → DTPs (Web and Desktop) or tap DTPs in the mobile sub-nav to see every open DTP across all patients in the workspace. This is useful for a clinical quality review — how many open "non-adherence" DTPs do we have, and who are the patients?
