Clinical Interventions

Track a drug therapy problem through the strategy you plan, who's doing it, and what happened.

An intervention is a drug therapy problem worth tracking to a resolution — a dosing issue, a possible interaction, non-adherence, an adverse reaction — logged once, then worked through strategies, team assignments and a recorded outcome.

Starting one

From the patient record's Interventions tab, or the standalone Clinical Interventions page, select Log intervention. You'll pick a category, a priority, and describe the issue (at least 10 characters — enough to explain what you found and why it matters).

Before you save, if this patient already has another open intervention in the same category logged in the last month, you'll see a warning naming it. It's just a warning — nothing stops you from continuing if this genuinely is a separate problem.

An intervention also gets logged automatically when a clinical encounter's decision is "medication-related intervention" or "chronic disease intervention" — it starts as a draft you flesh out from here, linked back to the encounter it came from.

Working through it

Every intervention has a status — Identified → Planning → In progress → Implemented → Completed, or Cancelled from any point. You move it forward yourself with a button; nothing advances automatically just because you added a strategy or assigned someone. That's deliberate — the record only shows what you actually decided happened.

Strategies are the specific actions you're taking — a dose adjustment, switching to an alternative, additional monitoring, a physician consultation, and so on — each with why you're doing it and what you expect to happen. Add as many as the problem needs.

Assignments hand part of the work to a team member — a role, a task, and their own status separate from the intervention's overall one. Whoever's assigned gets flagged on the workspace's alerts until they pick it up.

Outcome is recorded once you've implemented your strategy: how the patient responded, and which of the measurable wins happened (problem resolved, medication optimized, adherence improved, and so on) — plus optional cost savings and a patient-satisfaction rating. You need to record an outcome before you can mark the intervention Completed.

Follow-up creates a real follow-up task (the same kind you'd see under Follow-ups) with its own due date, so it shows up on the ordinary follow-up worklist rather than living only inside the intervention.

Audit trail

Every action on an intervention — created, status changed, a strategy added, an assignment made, an outcome recorded — is logged. Scroll to the bottom of the record to see who did what, and when.

Permissions

Everything here sits behind the Clinical Interventions permission line — View to see them, Create to log new ones, Edit for strategies/assignments/status/outcome/follow-up, Delete to remove one (it goes to the Recycle Bin, same as everywhere else).
